Senator Tony Mendoza invites you and your family to participate in a Free Health & Covered California Enrollment Fair
Join us for this FREE event to sign up your family for health insurance.
WHAT YOU WILL NEED TO BRING:

Current income of all family members on the application (copy of last year's Federal Income Tax return, W-2 form, two current pay stubs, etc.)
Legal residency or immigration documents
Copy of SSN (only if you are applying for coverage) and DOB for each family member in the household

OPEN ENROLLMENT ENDS ON January 31, 2016. Most people are required to have health insurance or pay a penalty on their tax returns. Tax penalties in 2016 will be higher than the year before.
